Back to School Physical

Back to school physical for school-aged Meriden residents will be done at the Meriden Health Department, 165 Miller Street, on the following days: Tuesday August 23rd, Wednesday August 24th, and Thursday August 25th. The cost of the physical is $20.00 per child or $50 for 3 or more children. School physicals are required to enter Pre-K, Kindergarten, 6th grade and 10th grade.Please call 203-630-4234 for an appointment. Bring your child's immunization records. Any missing immunizations can be given during the physical.
